Program aims to reduce residents’ hospital admissions

A new program will see specialist nurses attend residential aged care facilities in an effort to reduce the number of residents admitted to hospital emergency departments. Several major health services will participate in the Residential in Reach program, which will identify and provide treatment to at-risk residents.
