Program reducing emergency department admissions

A pilot program for aged care residents experiencing non-life threatening, acute health issues has resulted in a dramatic decrease in hospital emergency department presentations. The Geriatric Rapid Acute Care Evaluation (GRACE) program organises nurses and specialist equipment to assist in the care of residents who would otherwise go through the disruptive hospital admission process.
